Office Hours

Our office hours are Monday through Friday 8:00am to 5:00pm. (Closed for lunch from 12:00-1:00pm daily).

Prescriptions and Refills

Please bring your medications or a complete list with you to each appointment. If you need prescription refills, we prefer that you contact your pharmacy.

Please be sure to give us at least 48 hours notice on all refill requests.

Pain medication will not be refilled after hours or on weekends.

Some chronic or long-term medication refills require an office visit before they may be refilled.

After Hours Emergencies

If you have an urgent matter after hours and need to speak to the physician on call, please dial (337) 367-1444. An operator will answer your call and page the physician.

If you experience a life-threatening emergency, PLEASE DIAL 9-1-1 IMMEDIATELY.

Medical Records

A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider, we ask that you supply us with their name, address, and phone number.

We require a minimum of 48 hours to process records requests, unless it is for an urgent situation.
