Payments and Billing

Thank you for choosing us as your healthcare provider. We are committed to your treatment being successful. Please understand that payment of your bill is considered part of your treatment.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs.

We accept cash, checks, Visa, Mastercard, American Express, and Discover.

Questions About Your Bill ? - If you have a question about your bill, you may contact our Billing department, during regular office hours, at (337) 367-1444.

Accepted Insurance Plans

For insurance plans where we are a participating provider, all co-pays and deductibles are due at the time of service.

We will submit claims to your insurance carrier. However, in the event that services are not covered, the balance will be the responsibility of the patient or guarantor.
